Student Immunization Information

It is MN state law that immunization records be up-to-date before a child attends school.

REMINDER for incoming KINDERGARTENERS:
Minnesota Law requires students to have documentation of the following immunizations before they are allowed to begin Kindergarten:
DTAP: 5 (unless 4th was given after the age of 4)
Polio: 4 (unless 3rd was given after the age of 4)
MMR: 2 doses
Hepatitis B: 3 doses
Varicella (chicken pox): 2 doses or documentation of disease history provided by a health care provider.

REMINDER FOR incoming 7th GRADERS:
Minnesota Law requires students to have documentation of the following immunizations before they are allowed to begin 7th grade:

DTaP and Polio: at least 3 of each
MMR (Measles, Mumps and Rubella): a 2nd dose given after the first birthday
Tdap (Tetanus and diphtheria): 1 dose given after the seventh birthday
Hepatitis B: a series of 3 shots given over a 6-month period(Must have 1st shot to start school)
Varicella (Chickenpox): 2 dose, or documentation of disease history provided by a health care provider.
Meningococcal: 1 dose at age 11-12

**NEW REQUIREMENT: Seniors (12th graders) are required to have a second dose (booster) of the Meningococcal vaccine. 

Your student will not be able to begin school until their immunization record is up-to-date.
